This involves person centered recovery planning and collaborating with other services and agencies.Shifts Available:Full Time; AM/Swing 8:00 am - 4:30 pm; Sunday - ThursdayFull Time; Swing 11:00 am - 7:30 pm; Tuesday - SaturdayExpected starting wage range is $67,724.80 - $77,800.84. The full wage range goes up to $92,607.32. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Mobile Crisis Team to assess risk and provide additional support to individuals who can safely avoid a psychiatric hospitalization with appropriate support and resource referrals. The Transition Service serves individuals who are working towards discharge from an E&T or other housing setting and who could use additional support in successfully sustaining their transition to outpatient services (i.e. interim prescribing support; connecting with outpatient mental health services; seeking housing, employment or education opportunities; connecting with others in social settings, and other supportive services).
