Job Description:

Function: Leads educational, recreational, and wellness groups with adult and youth populations. Works with outpatient team and recreational therapist to develop programing that will benefit our clientele. Plays an integral role in the agency goal of increasing support services for clients. Serves as a member of an organization that welcomes, engages, and serves individuals with complex wellness needs.

  • Work with Clinical Program Manager and Recreational Therapist to identify and develop group curriculum.
  • Responsible for facilitating a minimum of 10 groups/week
  • Groups will focus on activities of daily living, socialization opportunities, education and support
  • At least 3 groups a week will take place at Progress House in their Club House Program
  • Responsible for co-facilitating Stride Program with clinical and medical staff
  • Responsible for engaging new consumers in group services by communicating opportunities and supporting their attendance
  • Responsible for documenting a client's participation in group within 24 hours of service delivery
  • Complete National Outcome Measures (NOMs) client-level in-person interviews and document answers in clients' electronic health records
  • Serve as part of a multi-disciplinary team providing advocacy and support to consumers with behavioral health needs
    Requirements
    • All new hires will be required to show proof of COVID-19 vaccination
    • Must have experience in case management or be a certified peer.
    • Experience in a behavioral health setting preferredHours
