Registered Nurse (RN) - Crisis Triage Facility  (Nights 36hrs/wk.; Thursday-Saturday)

As a Psychiatric Nurse, you will be responsible for conducting psychiatric assessments; assessing physical health needs; making appropriate referrals to community physicians; providing management and administration of medication in conjunction with the psychiatrist; providing a range of treatment, rehabilitation, and support services; and sharing shift management responsibility with other staff.   • Examine and assess clients upon admission, determine if the client is medically stable in order to provide care within the scope of practice of 24-Hours Services at KMHS.
  • Obtain admission orders from a physician within one hour of admitting, after obtaining pertinent information and initiating treatment orders.
  • Communicate with providers, process their orders, and implement them maintaining the highest level of nursing care.
  • Document client care, medication, and treatment administration. Evaluate, monitor, and document client responses to medication, treatments, progress to mental health goals, and plan of care.
  • Write and implement a nursing care plan upon admit. Review and/or revise every 24 hours. Collaborate with team (ie: Providers, MHPs, Shift Supervisors, etc) to establish and obtain pertinent goals.
  • Administer medications and treatment in a safe manner in accordance with standardized nursing guidelines Organize medications for administration as directed by the designated providers.  Dispose of medications according to KMHS protocol. Count and order medication for clients, maintain house stock, and count and document controlled medications per shift.
  • Initiate emergency procedures using technical skills and professional judgment and perform appropriate nursing/medical intervention(s).
  • Obtain provider orders, verify and initiate, including seclusion/restraint orders per WAC 246-337-110.
  • Train, educate, orient, and mentor new nursing staff, on-call nurses, and nursing students.
  • Provide nursing care including interpreting laboratory results, responses to medications, vital signs, and ongoing physical assessment every shift.
  • Delegate the non-nursing tasks to support staff.  Coordinate with the shift supervisor for unit safety and other client needs.
  • Participate in discharge planning, review treatment goals, assist in client medication educational needs, order discharge medications, etc.
  • Participate in program activities as a team member, including but not limited to: leading wellness groups, participating with clients on the milieu, or other interactions as needed.
  • Complete nursing notes, physician orders, and other documentation necessary, for quality care in an accurate, timely manner. While meeting agency, statutory, regulatory, statistical, and funding requirements.
