Company: Providence at Home with Compassus Position Summary The
Nurse Practitioner (NP) provides hospice and palliative care
services under the supervision of a hospice physician. This role
conducts home visits, performing physical, psychosocial, and
functional assessments to support prognosis determination and
ongoing care. The NP collaborates with the interdisciplinary team
to provide symptom management, counseling, and education to
patients and families, ensuring continuity and quality of care
throughout the hospice experience. Position Specific
Responsibilities • Perform home visits to assess disease
progression, including changes in clinical, cognitive,
psychosocial, and functional status. • Document assessment findings
and clinically relevant information related to patient eligibility
for recertification. • Communicate findings and care plan updates
to the hospice physician and interdisciplinary team. • Provide
assessments, goal setting, care plan revisions, counseling,
education, and symptom management in collaboration with the hospice
team. • Accurately document orders, care plan changes, education
provided, and other required details in the electronic health
record (EHR). • Complete documentation by 10:00 am the following
day. • Perform and document nursing interventions, including
patient and family education, adapting care as patient needs
change. • Maintain familiarity with Federal and State regulations,
Joint Commission standards, and organizational policies. •
Collaborate with physicians and the interdisciplinary team to
provide continuity of care. • Provide input into the development
and implementation of protocols, policies, and standardized
procedures. • Serve as a resource on pain and symptom management. •
Understand clinician schedules, time logs, and payroll accuracy
requirements. • Participate in quality improvement activities and
other duties as assigned. • Travel to patient sites as needed. •
Other duties as assigned. Education and/or Experience Required -
Master’s degree from an accredited Nurse Practitioner program,
Graduate of an accredited school of nursing, and Completion of all
NP program clinical requirements. Required: At least 1 year of
nursing experience in hospice or palliative care. Skills
Mathematical Skills: Ability to add, subtract, multiply, and divide
in all units of measure, using whole numbers, common fractions, and
decimals. Ability to compute rate, ratio, and percentage. Language
Skills: Ability to read, analyze, and interpret general business
periodicals, professional journals, technical procedures, or
governmental regulations. Ability to write reports, business
correspondence, and procedure manuals. Ability to effectively
present information and respond to questions from leaders, team
members, investors, and external parties. Strong written and verbal
communications. Other Skills and Abilities: Ability to articulate
and embrace integrated healthcare at home philosophy. Knowledge of
hospice regulations and recertification requirements under
physician direction. Strong skills in physical and psychosocial
assessment of patients and families. Effective verbal and written
communication skills in English. Ability to collaborate with
interdisciplinary teams while working under physician supervision.
Strong organizational and documentation skills, with the ability to
prioritize multiple tasks. Ability to maintain professional
boundaries and provide support to families in crisis. Competence
with electronic health record systems and basic computer
applications. Demonstrates knowledge of human development and
psychosocial needs across all age groups. Assesses and interprets
age?appropriate patient data to identify and meet specific needs.
Communicates effectively with patients and families across the
lifespan and connects them to community resources as appropriate.
Certifications, Licenses, and Registrations Required - State-issued
NP license. Certification: National Provider BLS – American Heart
Association required upon hire (if not already held). State
Specific Requirements Licensure: Active Nurse Practitioner License,
Active Registered Nurse License and Active Nurse Practitioner
Furnisher License. Certification: National Provider BLS – American
Heart Association required upon hire. Preferred: Certified Hospice
and Palliative Care Certification. Physical Demands and Work
Environment: The demands of this role necessitate a team member to
effectively perform essential functions. Adaptations can be made to
accommodate team members with disabilities. Regular standing,
walking, and manual dexterity are fundamental, along with the
ability to lift and move objects up to 50 pounds. Visual acuity
requirements include close and distance vision, color and
peripheral vision, depth perception, and the ability to adjust
focus. In a healthcare setting, exposure to bodily fluids,
infectious diseases, and conditions typical to the field is
expected. Routine use of standard medical equipment and tools
associated with clinical care is essential. This description

$77.91 The amounts listed are the base pay range; additional
compensation may be available for this role, such as shift
