This position involves caring for patients in the comfort of their homes across various locations, ensuring they achieve their highest levels of health and independence. Assess patients' condition during every visit and chart your observations Perform evaluation tasks, including vital signs and medication review Administer medication as prescribed by the patient’s physician Provide palliative care as needed to keep the patient comfortable Make recommendations for devices or tools that might improve the patient’s quality of life Educate patients and their families on proper home health care strategies and procedures Coordinate with Occupational Therapists, Physical Therapists, Physicians, and anyone else involved in the patient’s care plan Provide regular updates to the patient’s physician and registered nurse supervisor Proficient in maintaining accurate medical records Adept at multitasking, listening, and verbal communication Skills in providing home care or outpatient care Ability to handle physical workload Strong problem solving and critical thinking skills Knowledgeable in disease management and health promotion Medical Insurance Dental Insurance Vision Insurance Life Insurance Valid nursing license BLS/CPR certification Driver’s License and reliable transportation
