RN - Care Manager at Providence Health and Services summary:
Registered Nurse Care Manager responsible for clinical assessment,
care planning and coordination of oncology patients in
collaboration with physicians, pharmacists and multidisciplinary
teams. Provides patient and family education, psychosocial support,
survivorship planning and continuity of care across the treatment
continuum. Requires an active Washington RN license; oncology
experience preferred and position is full-time, on-site with salary
range provided. Description The Care Manager RN utilizes in depth
knowledge of physical assessment and management of illnesses or
conditions within the registered nurse's scope of practice. This
includes collaboration with other licensed health professionals
such as physicians, pharmacists, and other nurses. The Care Manager
RN will work directly with physicians on issues related to care
plans. In collaboration with the medical staff and Oncology
Administration, this position provides patient and family
education, psychological support, social support and survivorship
care planning throughout the continuum of care and services and

of a recognized program to obtain an RN license in the State of
Washington. Upon hire: Washington Registered Nurse License
Preferred Qualifications: 2 years Experience in Oncology nursing.
