When you work at Telecare, you help to empower thousands of individuals every day in their journeys of recovery.Telecare's Community Alternatives Team (T-CAT) provides the following outreach services in Pierce County: The Diversion Service works in partnership with Community Hospital Emergency Departments and Pierce Co. Mobile Crisis Team to assess risk and provide additional support to individuals who can safely avoid a psychiatric hospitalization with appropriate support and resource referrals. The Transition Service serves individuals who are working towards discharge from an E&T or other housing setting and who could use additional support in successfully sustaining their transition to outpatient services (i.e. interim prescribing support; connecting with outpatient mental health services; seeking housing, employment or education opportunities; connecting with others in social settings, and other supportive services). The Transition Service also receives referrals from many other community stakeholders to address an individual's emerging needs in the community such that a return to an acute hospital setting can be prevented wherever safely possible.Employment Status: Full-TimePOSITION SUMMARYUnder supervision of a licensed clinician, the Social Work Clinician II provides clinical and casework services to members served and natural supports. This involves person centered recovery planning and collaborating with other services and agencies.ESSENTIAL FUNCTIONSDemonstrates the Telecare mission, purpose, values, and beliefs in everyday language and contact with internal and external stakeholdersProvides safe, effective, and efficient implementation of direct care in accordance with established policies, procedures, and standards of careWorks with members served and multidisciplinary treatment teams to design, implement, and evaluate the Recovery PlanUnder clinical supervision, establishes and maintains a therapeutic relationship with members servedEnsures provision of needed clinical services to assigned members served; coordinates ongoing treatment within the program and with outside agenciesProvides individual and group therapyDocuments all treatment provided in a thorough and consistent mannerCollaborates with other staff in the discharge planning process and makes linkages to community services for members servedActively participates in multidisciplinary team meetings and recovery planning meetingsDemonstrates knowledge of de-escalation techniques and crisis communication and managementServes as a clinical consultant to recovery team membersMust evade members served in the event of assaultive behavior and pass assault crisis/crisis prevention trainingDuties and responsibilities may be added, deleted, and/or changed at the discretion of management.Position requires transporting Members in program van (acceptable driving record) and driving for company business such as trainings, offsite meetings, etc.QUALIFICATIONSRequired:A Master's Degree in Social Work from an accredited graduate school and working towards licensure and registered with the Board of Behavioral ScienceOne (1) year of direct service experience with individuals with severe mental illness in an inpatient or outpatient setting is requiredCriminal justice clearance and education verificationMust be at least 18 years of ageMust be CPR, Crisis Prevention Institute (CPI), and First Aid certified on date of employment or within 60 days of employment and maintain current certification throughout employmentAll opportunities at Telecare are contingent upon successful completion and receipt of acceptable results of the applicable post-offer physical examination, 2-step PPD test for tuberculosis, acceptable criminal background clearances, excluded party sanctions, and degree or license verification. Position requires driving, valid driver license, a motor vehicle clearance, and proof of auto insurance is required at time of employment and must be maintained throughout employment. Additional regulatory, contractual, or local requirements may apply.Preferred:Previous experience with population to be served and/or work in a similar program settingOne (1) year experience in drug and alcohol setting preferred; internship, employment, or volunteer workCADAC or similar certificationKnowledge and experience with SBIRTKnowledge and experience with ASAMFamiliar with mental health screening tools i.e. GAD-7, PHQ-9SKILLSAbility to complete social histories, risk assessments, mental health status exams, and plans of careAbility to work collaboratively with physicians, nurses, and rehabilitation therapistsAbility to form a therapeutic alliance with members served and familiesSufficient medical knowledge to assess the needs of members servedSkills in conducting group therapyAbility to advocate for the members served in other health delivery systemsPHYSICAL DEMANDSThe physical demands here are representative of those that must be met by an employee to successfully perform the essential functions of this job.The employee is occasionally required to sit, stand, walk, bend, push, pull, squat, kneel, and lift and carry items weighing 25 pounds or less as well as to frequently reach, twist, and do simple and power grasping.
